Your First Appointment

An Overview

In a typical first visit, we will already have your new patient forms from you (by e-mail, fax, mail), as well as your insurance information. Using that information, we will have your insurance file setup.

The steps below are what you can expect at a typical first visit at our office. Please note that you may not be able to have a "regular cleaning" at your first visit, depending on the condition of your gums / bone / teeth. A "scaling and root planing" may be necessary if you have a perio (gum) condition or disease. If that's the case, we will explain your condition fully and offer options to improve and manage your perio condition.

    A typical appointment:

  • full set of digital x-rays taken
  • prophylaxis (professional cleaning)
  • polishing / flossing
  • homecare instructions provided
  • dentist exam and discussion
  • treatment plan set with patient
  • next appointment scheduled
  • payment and insurance benefits review

New Patient Forms

Before your first visit, please print and complete this New Patient form.

Please be sure to sign it after printing the fillable PDF form.
Then, please e-mail the forms to contact@jamieparkdds.com or "snail mail" the signed form to us, so we can research any insurance plan you would like to use.

Additionally, please review our HIPAA privacy form fully before completing the New Patient forms.

PPO Dental Insurance

Did You Recently Take Dental X-rays?

Most new patients usually take x-rays at our office during their first visit.

However, we may be able to use recent x-rays you may have taken at your former dental office. However, they must be less than 12-months old, and they must be transmitted to us in .JPG format.

If you have x-rays that are older than 12-months, we will not be able to use them because so much change can occur within that time. We would be doing a disservice to you by relying on old x-rays.

Also, if we do not receive x-rays by your appointment time, we will take new x-rays so we can diagnose you properly during your visit. Your former dental office will typically need at least 48-hours advance request to get the x-rays sent out in time for your appointment.

Please keep in mind that our digital x-ray devices emit very low radiation, so avoiding radiation exposure is not much of a concern for patients at our office.

Our practice philosophy is to provide the best in restorative dentistry, with a solid financial path, in a comfortable and modern facility.
